3rd DAY (16th Febuary 2011)

Today morning we departed to a place called Kampung Padang Gelam for an aedes larval survey due to a case reported yesterday via e-notification from Hospital Jitra.We surveyed from house to house to look for mosquito larvae and possible sites of breeding.We also saw the personnel putting Abate into the watertub to prevent the breeding of aedes mosquito. We managed to find the larvae in a bucket containing stagnant water in one of the houses. Discovering larvae for the first time in life was truely an unforgettable experience.Besides, we also saw a traditional way of preventing mosquito by rearing Guppies in the well.

There are certain rules to adhere to when we survey a house. Firstly, we must introduce ourselves to the house owner by showing our ID and explain to them that we are from Ministry of Health. Next we must ask permission from the owner of the house to let us enter and inspect their house for possible larvae breeding area. When we enter their house, the owner must escort us around to avoid false accusations of theft.

When we discover larval breeding area in the house compound, we must inform the owner and show him the site.Besides,before collecting the larvae, we must show the owner the empty container we are going to use . These is to ensure that we do not bring any larvae from any other location.After collecting the sample, the owner has to fill in a form of consent  for investigation.Then the larvae is brought to the lab for further investigation (ENTOMOLOGY).

After the survey we headed to the headquaters for a small discussion regarding the protocols in managing the cases of dengue.First, we discussed regarding the organisation of the VEKTOR group. Then we discussed how to overcome the dengue outbreak around the community. For your information, during 2009- all suspected cases were considered as dengue cases. After 2009, they set a new rule whereby a suspected case will be verified before considering as a case od Dengue fever. Besides, 2 cases occuring  within 2 weeks in that particular area will be consider as an OUTBREAK!!!

There few ways of notifying the cases from the hospital to the health care. Currently they are using the e-dengue and VEKPRO as the method to record all the notifications they recieved.Using VEKPRO, we can only know the local epidermiology of dengue cases. However with e-dengue we can see the epidermilogy of dengue through out  Malaysia.

If there are no cases being notified from the hospital, the personnel from the Ministry of Health will conduct a survey by using Aedes Index(AI), Breteau Index(BI) and Container Index(CI).


Aedes Index(AI)= Premis positive with larvae/Premis being inspect x 100%
               Normal = (0%)

Breteau Index(BI)= Container positive with aedes/Premis being inspect x 100%
               Normal  = (<5%)

Container Index(CI)= Container positive with larvae/Container being inspect x 100%
                Normal    = (0%)
If the values exceed the normal range,fogging will be carried out at that particular area. Besides,education on breaking the chain of dengue outbreak will be conducted  within the community. In addition, gotong-royong will also be carried out at that particular area. The Health Ministry sponsors the torch lights and t-shirts for the activity. Furthermore,COMBI will be formed among the community whereby it  is a group of people from the community who are selected to represent them. They will be given education regarding Dengue fever by the health officers. Then, it is their responsiblity to educate the villagers.
